TherapyNotes Alternatives: An Honest Comparison for 2026

I'm going to tell you something that TherapyNotes' marketing team won't.

TherapyNotes is solid. It is built only for behavioral health, its insurance billing is strong, and its per-claim price is one of the lowest around. You can call and reach a person at any hour. They have been doing this for about two decades.

But a solo plan costs more than most of the alternatives, and if you want AI to draft your notes, that is a paid add-on on top. For a therapist who mostly wants to stop typing at 9 PM, the math starts to look different.

I'm Albert Wong. Psychologist. Built Practice Harbor because I spent five years writing progress notes at 9 PM and two years watching therapists scroll past three EHR options and land back on TherapyNotes because "I know it works."

Here's what I actually think. For the right practice, TherapyNotes is still the answer. For a lot of others there is a better fit now. Below are the four alternatives worth a look, what each costs, and who should stay put.

The short answer

The main TherapyNotes alternatives for therapists are SimplePractice (from $49/month), Sessions Health ($39/month), Jane (from $59/month), and Practice Harbor ($19/month with AI notes included, free for pre-licensed clinicians). TherapyNotes itself is $69/month for a solo clinician and remains a strong choice for practices that file a lot of insurance claims or want 24/7 phone support.

TherapyNotes Alternatives Compared

Prices are each vendor's published monthly price for one clinician, before any introductory discount. Check the vendor's pricing page before you decide; these change.

SimplePractice, Sessions Health, and Jane prices are from each vendor's pricing page as of October 2026 (Jane in USD). TherapyNotes figures are as of September 2026.

SimplePractice

The category's default and the one most therapists compare first. Three solo plans at $49, $79, and $99 a month, with an AI Note Taker sold separately at $35 a month. If you are weighing it, we wrote a full SimplePractice alternatives guide and a SimplePractice vs TherapyNotes comparison.

Sessions Health

A lean, therapist-focused option: $39 a month for the first clinician and $29 for each additional one, with a free tier for up to three clients. Telehealth ($10 a month) and the AI Assist suite ($35 a month) are add-ons. More in our Sessions Health write-up.

Jane

Well designed, and used across many health disciplines rather than therapy alone. Plans run $59, $79, and $99 a month, and the lowest one is capped at 20 appointments a month. AI Scribe is a $15 a month add-on. See our Jane comparison for therapists.

Practice Harbor

Ours. $19 a month for a licensed clinician, free for pre-licensed clinicians, and $39 a month for practices with a team. AI progress notes, telehealth, and the client portal are part of every plan rather than add-ons, and claims are $0.25 each. The line-by-line version is on the Practice Harbor vs TherapyNotes page.

Where TherapyNotes Actually Wins

Let's start here. I'm not going to pretend they're bad at things they're good at.

Per-Claim Price

At $0.14 a claim, TherapyNotes is cheaper per claim than we are ($0.25) and cheaper than Sessions Health. If you are a group filing thousands of claims a month, that difference is real money and you should run the numbers both ways.

Phone Support, Around the Clock

TherapyNotes offers 24/7 phone and email support. Ours is founder-led and email-first. If you want to pick up a phone at 2 AM and talk to someone, that is a genuine advantage for them.

Track Record and ePrescribe

Two decades serving behavioral health only is worth something, especially to a large practice that cannot afford surprises. TherapyNotes also sells an ePrescribe add-on ($65 a month), which matters if you have prescribers. Practice Harbor does not offer ePrescribe.

Why Therapists Look for an Alternative

The Solo Price

$69 a month is at the high end for one clinician. A private-pay therapist who files few or no claims is paying for a billing engine they rarely touch.

AI Notes Cost Extra

TherapyNotes' AI, TherapyFuel, is a $40 a month add-on. So a solo clinician who wants AI-drafted notes is at $109 a month. That is $1,308 a year before a single claim is filed.

Pre-Licensed Clinicians Pay Too

Associates and trainees are billed as clinicians. For a supervisor with three associates, or an associate paying their own way, that adds up fast.

What Practice Harbor Does Differently

AI Notes Are Built In, Not Bolted On

Record a session, dictate, or type a few lines, and Practice Harbor drafts the progress note in SOAP, DAP, BIRP, GIRP, or a template your practice writes. You review, edit, and sign. Nothing is filed without your signature.

Transcription and drafting run under Business Associate Agreements with zero data retention, and session audio is never used to train models.

Clients Do the Intake

Clients sign in to the portal to complete intake forms, sign consents, see and request appointments, join telehealth, message the practice, and pay balances or keep a card on file. That is time you are not spending on email and paper.

Free for Pre-Licensed Clinicians

You're writing real clinical notes. Your supervisor is reviewing and co-signing them. The software shouldn't cost anything, and it doesn't: pre-licensed clinicians are free on every plan. When you get licensed, it's $19 a month. Same product.

Your Data Is Always Yours

Records are stored in the FHIR R4 standard, not a proprietary schema, and everything exports at any time in open formats. A company that's confident in what it builds doesn't make leaving hard.

The Honest Limitations

Practice Harbor isn't TherapyNotes. And I'm not going to pretend it is.

Who Should Stay With TherapyNotes

  • You file a high volume of claims. At thousands of claims a month the per-claim price can outweigh the subscription difference. Do the arithmetic with your own numbers.
  • You want phone support at any hour. They have it. We don't.
  • You need ePrescribe in the same system.
  • You write your own notes and plan to keep doing so. Then the AI add-on price doesn't matter to you, and switching has to pay for itself some other way.

Who Should Try an Alternative

  • You spend your evenings on notes. AI drafting is the biggest single change, and on TherapyNotes it costs $40 a month extra. On Practice Harbor it's included.
  • You're mostly private pay. You don't need to pay a premium for a billing engine. Sessions Health or Practice Harbor will cost you less.
  • You're pre-licensed, or you supervise people who are. Practice Harbor is free for pre-licensed clinicians, with note co-signing and supervision hours logging built in.
  • You want a calendar-first, multi-discipline clinic tool. Look at Jane.
  • You want the most familiar mainstream option. Look at SimplePractice, and price the plan and add-ons you would actually use.

How to Switch From TherapyNotes Without Disrupting Your Practice

If you're thinking about moving, do this. In this order.

  1. Export everything from TherapyNotes first. Notes, client records, billing history, treatment plans. Keep an encrypted copy. Don't close the old account until you have it, and check your state's record-retention rules before you delete anything.
  2. Try the new system for a week. Create a sample client. Write one note. Book one telehealth call. You'll know whether it fits.
  3. Bring your records over. Practice Harbor imports client demographics, notes, treatment plans, and documents from a previous EHR's export.
  4. Overlap for two weeks. Add new clients to the new system and keep seeing existing ones in the old one until you're confident.
  5. Tell your clients. "We're moving to a new portal. Same therapist, new tool. Here's your new link." They care that you're still there.

The Bottom Line

TherapyNotes is a solid, insurance-focused EHR with a long track record. For a claims-heavy group that wants phone support, it has earned its place.

But it's not the cheapest way to run a solo practice, and AI notes cost extra. If that's what's bothering you, the alternatives above are worth an afternoon.

Practice Harbor is $19 a month for licensed therapists and free if you're pre-licensed, with AI notes, telehealth, the client portal, and claims included. If you spend your evenings typing notes, try it.
